• The Parent/Legal Guardian acknowledges and recognizes that OMNI cannot provide any supplemental or specialized care to the student based on the student’s condition.
• The Parent/Legal Guardian acknowledges and recognizes that OMNI can only provide the scope of services as is provided for other children at OMNI.
• In addition, the Parent/Legal Guardian recognizes and understands that the student will be exposed to other children from various ages in a group setting and will be exposed to illnesses and conditions normally found in children.
• The Parent/Legal Guardian acknowledges that OMNI does not have the facilities or personnel sufficient to provide any specialized care to the student and that because of the child’s special condition, said student would not be permitted enrollment in OMNI International School without this Waiver and Release of Claims.

I authorize and grant permission for OMNI International School to contact, request or seek a physician or any appropriate individual in the event of an emergency, whenever or wherever an emergency occurs, to administer and provide any medical care deemed necessary by OMNI International School. I also authorize and agree that OMNI International School may administer and provide necessary or required emergency medical care for the student in the event said medical care is deemed necessary by OMNI International School. The Parent/Legal Guardian hereby releases and forever discharges OMNI International School and its employees or agents from any liability arising in law or equity as a result of OMNI’s employees or agents administering emergency care (i.e. epi-pen, inhaler, first aid, CPR) and providing other emergency care in align with the information delineated on the student’s special conditions below. This release supersedes and replaces all prior negotiations and all agreements proposed or otherwise, whether written or oral, concerning all subject matters covered herein. This tool, in addition to the Authorization constitutes the entire agreement among the parties with respect to the subject matters discussed herein. The reference in this Release to the term OMNI shall include OMNI’s affiliates, successors, directors, officers, employees, and representatives. The terms Parent/Legal Guardian shall include the dependents, heirs, executors, administrators, assigns, and successors or each.

I hereby give OMNI International School authorization to allow photographs and videos of my child to be used for publicity purposes including but not limited to brochures, advertising, PR campaigns, fundraisers, internal newsletters, websites, etc. As always, any footage used will be in the best possible light for the student and school as a whole.
